专利说明:
FIELD OF THE INVENTION The invention relates to medicine, in particular to surgical disintegrating apparatus. The purpose of the invention is to reduce tissue trauma when the apparatus is removed after suturing by moving the die relative to the body. Fig. 1 shows a surgical stapler, an axonometric projection 4HHf in Fig. 42, the same longitudinal section; on fig.Z - working part of the binder, a longitudinal section; figure 4 is the same, the second option with a partial digging; figure 5 - section aa in figure 4; FIG. 6 is a surgical stapler, the third option, a general view; FIG. 7 is the working part of a surgical stapler according to the third option, a longitudinal section. The surgical stapler includes a housing 1 with a die 2 and a pusher 3, a magazine 4 for placing sko (5о 5, a mechanism for feeding brackets 5 to the die 2 and a drive for the pusher 3. The stapler is provided with a cantilever holder 6, on which working matrix there is a reinforced matrix 2, The cantilever holder 6 is pivotally mounted on the housing 1 so as to move the die and has a surface-to hold the central portion of the bracket 5. On the stapler there are handles 7, pivotally connected to the drive element 8, on which the resilient plate 9 with the protrusion 10 is mounted. In the case 1 there is a cam I with a side surface 12. On a thickness of -. The lug 3 is made protrusion 13. A passage 14 is made along the body 1. A cam element 15 is located on the drive element 8, and a cam element 16 on the console holder 6. The console element 6 is spring-loaded with a flat spring 17. A recess 18 is made on the cam 11. and the drive element 8 is attached to the return spring 19. The mechanism for feeding the brackets is made in the form of a figure channel, in which brackets 5, bracket pushers 20 and springs 21 are installed. The device is used in the following way. To activate the stapler, the surgeon grabs him by the handles 7 and squeezes the latter towards each other, moving it. herewith, the drive element 8 from the initial position before engagement with the pusher 3. Thereafter, the drive element element 8 moves pusher 3 from the loading position towards the pushing position. As soon as the drive element 8 is engaged with the pusher 3, the second protrusion 10 located on the elastic plate 9 engages with the first end surface of the cam 11, which, as a result of its cam action, moves the second protrusion 10 to the first side surface 12 of the cam 11 and disengages it from the protrusion 13 located on the pusher 3 After this drive element 8 pushes the pusher 3 along passage 14 until it engages with one of the wire brackets 5 fed into the inlet opening of the passage 14. Then the plunger moves wire brace 5 along the aisle 14. Simultaneously cam element 15, located on the drive element 8, disengages from the cam element 16 located on the cantilever holder 6. In this case, under the action of the spring 17, the cantilever holder 6 moves, and with it the die 2 also moves to wire bracket in passage 14. As soon as the pusher 3 moves the wire brace 5 to the position in which it is pressed against the matrix 2 and its parallel end portions exit the exit orifice, the protrusion 10 falls into the recess 18. In the event that the surgeon at this point stops compressing the handles of the stapler , the drive element 8 and the pusher 3 remain in this intermediate position. In this case, the surgeon has the opportunity to insert the ends of the wire brace 5 into the material J to be stapled with brackets (in particular, the B edge of the torn tissue) 5, more carefully straighten the material to be stapled before stitching the knobs 7 and finally, transfer the pusher 3 to its working position, in which the wire bracket 5 is folded in the form of a closed loop, which seizes the sewn fabric. After that, the surgeon may hold the knobs 7 of the stapler in the compressed state if it is necessary to keep the folded wire brace 5 near the matrix 2 at the end of the stapler. In this case, it is possible to hold or move the tissue fastened with a wolf brace. In the event that the surgeon removes the knobs 7 of the stapler and allows them to return to their original position under the action of the spring 19 and the elasticity of the knobs 7 themselves, the cam element 15 again engages with the cam element 16. As a result, the console holder 6 of the matrix 2 moves to the direction opposite to the clamping action of the spring 17 and, therefore, the matrix 2 leaves the wire clip 5 bent in a closed loop view. Thus, it is not necessary to move the binder body 1 for this purpose. At the same time, the protrusion 10 moves along the second end surface and the second side surface of the cam 11, which leads to the engagement of the protrusion 10 with the protrusion 13 located on the push rod 3. As a result of the engagement of the protrusion 10 s. the projection 13, the pusher 3 moves to its loading position simultaneously with the return of the drive element 8 to its original position. The proposed stapler can be made according to the second variant (Fig. 8; And 9). The same elements of this binder have the same numerical designation with the addition of the letter a. This stapler is similar to the stapler in the first embodiment, but the cam element 22, which interacts with the cam element 16o | located on the cantilever holder 6a, is fixed on the Za pusher and not on the Za drive element. The matrix 2a on this crosslinker, as well as on the crosslinker of the first embodiment / is mounted on the cantilever holder 6o. The proposed stapler can also be made according to the third variant (FIGS. 5 and 6). Elements of the same designation have the same designation with the first variant with the addition of the letter b, the Matrix 2 on the crosslinker according to the third variant, as well as on the crosslinker according to the first variant, is movable on the console holder 6b. The stapler is also provided with cam elements 23 and 24 located on the drive unit. A handle 26 is mounted kinematically on the housing 1b of the binder 25, which is kinematically connected to the pusher. For ease of use, the grooves 27 are made on the housing 1b and on the handle 26. The handle 26 is fitted with a plate 28 with an elastic protrusion 29, and a guide 30 in the case 1b to eliminate the spontaneous opening of the housing 1b and the handle 26 and move the pusher 3b in the opposite direction . 14 J59l3fOln S 12 FIG. 2 ) P / e V : XI - -) jiura Behind Fig 5 Js FIG. 7
